Initial Visit
- Consultation
We provide a consultation at your initial visit to get to know you personally, provide a thorough examination and diagnosis, and review treatment options. Dr. Sullivan utilizes his background in both general dentistry and endodontics to give individualized treatment options that take into account your overall and oral health. Our team will take digital x-rays of your tooth and Dr. Sullivan will review them with you in detail to educate you on your diagnosis and treatment options. We will review options for sedation and nitrous oxide if you are nervous about treatment. We want you to be completely comfortable with any recommended treatment and take the time to answer any questions. Once all of your questions have been answered, we will get you scheduled for any necessary treatment. Your personal health information will be kept private in accordance with HIPAA Privacy Regulations. - Please assist us by bringing the following to your consultation
–Your referral slip
-Any relevant X-rays- additional X-rays will be taken as needed
-A list of any medications, medical conditions, and allergies
-Dental insurance card if you have dental insurance
-A parent or guardian must accompany all patients under the age of 18
-Please let our office know if you have a medical condition that may be of concern (i.e. diabetes, high blood pressure, history of bisphosphonates, history of heart attack or stroke, artificial heart valves, artificial joints, rheumatic fever, etc.)
